Dear Healthcare provider,
I am writing this letter in an effort to collaborate with you with the intention of getting the best help and support for my medical condition. I understand that time is of the essence within our medical model, so I am going to be brief and to the point.
I have an eating disorder, and despite what you might assume about my health condition by looking at my body, it is imperative to my medical care that my eating disorder is taken seriously.
Because of this, I ask that you follow through with the following non-negotiable pieces of my medical care so that I am not at risk of harm or relapse.
Please do not weigh me unless it is absolutely medically necessary- in the case of anesthesia dosage or another specific medical procedure where this is needed. In the case of medical necessity, Please ask me to step on the scale backward and alert the MA and admin staff to take care to black out the number on all follow-up documentation printouts with a black heavy sharpie. Please do not put my weight into my electronic medical chart.
Please do not tell me to lose weight. The pursuit of weight loss is detrimental to my health and well-being. Please treat me for what I have come in for, evaluate for my concerns, and do not prescribe weight loss.
Because I am in eating disorder recovery, I see a dietitian regularly. I discuss all food and nutrition concerns with them, and so I ask that if you have any food and nutrition ideas about my medical concern, I will give you their contact information and you can speak with them directly.
